Tadalafil: More than just Erections

Tadalafil is best known by its brand name, Cialis, and by its nickname, "the weekend pill." That nickname undersells it. Over the past two decades, tadalafil has become an approved treatment for several conditions, and it is the only drug in its class approved for low-dose daily use in men. Recent large population studies have also linked it to possible heart and brain health benefits, which has drawn a lot of attention. This article covers the approved uses, how standard and daily dosing differ (following Health Canada's approved dosing), what the newer research shows, and safety points every patient should know.

How Tadalafil Works

Tadalafil belongs to a class of drugs called phosphodiesterase type 5 (PDE5) inhibitors, along with sildenafil (Viagra), vardenafil and avanafil. By blocking the PDE5 enzyme, it raises levels of a signalling molecule called cGMP. That relaxes smooth muscle and widens blood vessels in the penis, the prostate and bladder region, and the lungs [1][2].

What sets tadalafil apart is how long it lasts. Its mean half-life is about 17.5 hours, compared with roughly 4 hours for sildenafil. Blood levels peak at a median of about 2 hours after a dose [1][3]. That long half-life explains both of its well-known features: one dose can work for up to 36 hours, and a small daily dose keeps a steady level in the body. With daily dosing, steady blood levels are reached within about 5 days [17].

The Benefits of Tadalafil

Benefit 1: Erectile Dysfunction (ED)

Erectile Dysfunction was the first approved use for tadalafil (U.S. approval in 2003) [1]. In Canada, the standard on-demand dose is 20 mg, and it works from about 30 minutes up to 36 hours after a dose [17]. That wide window takes much of the pressure off timing of intimacy. Sexual stimulation is still needed to attain an erection. Tadalafil does not raise libido nor does not cure ED [1].

On-demand dosing vs. once-a-day dosing

For men who have sex regularly, or who would rather not plan around a pill, low-dose daily tadalafil (5 mg, or 2.5 mg if needed) is an option [17]. A 2019 meta-analysis in Sexual Medicine pooled 4 randomized trials (1,035 men) with at least 24 weeks of treatment. Compared with as-needed dosing, daily dosing produced :

  • A small but significant gain in erectile function scores (IIEF-EF mean diff 1.24)

  • Better penetration success (SEP2) and intercourse success (SEP3) rates

  • A lower overall rate of treatment-emergent side effects (OR 0.73)

  • No difference in stopping treatment due to side effects

  • In one included trial, 72% of patients preferred the daily regimen when given the choice [4].

Benefit 2: Benign Prostatic Hypertrophy (BPH) & Lower Urinary Tract Symptoms (LUTS)

Tadalafil 5 mg once daily is approved in Canada for BPH symptoms and for men who have both ED and BPH [17]. In the U.S., the FDA approved tadalafil 5 mg daily in October 2011 for the signs and symptoms of benign prostatic hyperplasia (BPH). It also approved it for men who have both ED and BPH. It is also used for men who have frequency of urination (those who pee too frequently) and with nocturia (those getting up to pee at night).

And the evidence is solid:

  • In a 12-week international randomized trial (325 men), tadalafil 5 mg daily cut the International Prostate Symptom Score (IPSS) by 5.6 points, versus 3.6 with placebo. Improvement began within the first week and was statistically significant by week 4. In sexually active men who also had ED, erectile function scores rose by 6.7 points versus 2.0 with placebo [6].

  • A 2021 meta-analysis of 15 trials (9,525 men), summarized by American Family Physician, found consistent improvements in total IPSS, urinary voiding and storage symptoms, and quality of life. It earned a Strength of Recommendation of A (That’s good).

  • Compared with tamsulosin (Flomax), tadalafil gave similar relief of urinary symptoms plus a clear benefit for erectile function. That makes it an attractive choice when both problems are present [7].

  • It is well tolerated, with no significant increase in serious adverse events compared to placebo.

  • Unfortunately this effectiveness appears to decline in men over 75.

  • Tadalafil improves how symptoms feel but does not significantly improve measured urine flow rate or leftover bladder volume [6][7].

Benefit 3: Pulmonary Arterial Hypertension (PAH)

Under the brand name Adcirca, tadalafil is approved in Canada for pulmonary arterial hypertension in patients with World Health Organization functional class II or III who have not responded to conventional therapy. The Canadian dose is 40 mg once daily [19]. In the pivotal 16-week trial of 405 patients, 40 mg daily increased the six-minute walk distance by 33 metres more than placebo. Clinical worsening occurred in 5% of patients on 40 mg versus 16% on placebo [2]. Tadalafil is now also available combined with Macitentan in a single daily tablet (Opsynvi, approved in the U.S. in 2024; check Canadian availability with your specialist) for adults with WHO functional class II–III PAH [8].

Benefit 4: Recovery After Prostate Cancer Surgery

Tadalafil is often used for "penile rehabilitation" after nerve-sparing radical prostatectomy. The REACTT trial randomized 422 men to 5 mg daily, 20 mg as needed, or placebo for 9 months after surgery. By the end of treatment, 22.3% of the daily group had returned to their pre-surgery erectile function category, compared with 11.3% on as-needed dosing and 7.8% on placebo [9].

There is an important caveat. After a 6-week drug-free washout, the differences between groups disappeared [9]. In other words, early daily tadalafil improved function while men were taking it and may shorten recovery time, but it did not clearly restore unassisted function once stopped [9].

Some Emerging Research: The Heart the Brain and Beyond

This is where most of the recent excitement is, and where caution matters most. All the findings below come from observational studies. They show associations, not absolute proof that tadalafil causes the full benefit.

Cardiovascular Outcomes and Mortality

  • Large observational cohorts (one following over 500,000 men) found tadalafil use was associated with roughly 30%+ reductions in all-cause mortality, heart attack, stroke, and venous thromboembolism compared to non-users (UTMB; PubMed). This has fueled interest from longevity-focused communities. But again, these are association studies subject to the same healthy-user bias. Meaning men able to take an ED drug are, on average, healthier to begin with — so this is not the same as proof the drug itself extends life or prevents heart attacks.

  • A 2024 meta-analysis in the European Heart Journal – Cardiovascular Pharmacotherapy pooled 16 cohort studies covering more than 1.25 million people. PDE5 inhibitor use was associated with a 22% lower risk of major adverse cardiovascular events (RR 0.78) and a 30% lower risk of death from any cause (RR 0.70) [10].

  • A large U.S. claims study of men with ED and cardiovascular risk factors (about 72,500 men in the matched cohort) found PDE5 inhibitor users had 13% fewer major cardiovascular events (HR 0.87), 25% lower overall mortality, and 39% lower cardiovascular mortality. The benefit appeared greater in men who filled the most prescriptions [11].

  • A 2025 study in the American Journal of Medicine looked at TriNetX records for men aged 40 and older. In men with ED, tadalafil was associated with lower risks of death (RR 0.66), heart attack (RR 0.73), stroke (RR 0.66), blood clots (RR 0.79) and dementia (RR 0.68). These associations were somewhat stronger than those seen with sildenafil [12].

Alzheimer's Disease and other Cognitive Disorders

  • A 2024 UK cohort study in Neurology followed 269,725 men with ED for a median of 5.1 years. PDE5 inhibitor initiators had an 18% lower risk of Alzheimer's disease (HR 0.82; 95% CI 0.72–0.93), with the strongest association in men issued more than 20 prescriptions. The authors called for a randomized trial to confirm the finding [13][14].

  • Small imaging studies also show tadalafil increases cerebral blood flow in some brain regions and modestly improved cognitive testing in men with mild cognitive impairment.

  • A 2024 meta-analysis of six studies found an overall association with lower Alzheimer's risk. However, the tadalafil-specific result was not statistically significant, and the authors rated the overall evidence quality as low [15].

The Contradicting Evidence:

  • A 2024 Mendelian randomization study, which uses genetics to test cause and effect, found no link between genetically lower PDE5 activity and Alzheimer's risk (OR 1.00). This adds a note of skepticism [16].

  • The NIH's own DREAM study — designed specifically to test this hypothesis using Medicare data — found sildenafil and tadalafil did not reduce Alzheimer's or related dementia risk and found no molecular correction of Alzheimer's-related abnormalities. A separate real-world cohort study found tadalafil specifically showed no association with dementia risk (HR 0.99, essentially no effect). A 2025 randomized trial in people with cerebral small vessel disease found 3 months of tadalafil did not improve cognition. And a genetic (Mendelian randomization) study — a method designed to sidestep confounding — found no causal link between PDE5 inhibition and Alzheimer's risk

  • These are concerning and mean more research is needed.

Heart failure and post-surgical exercise capacity — small positive trials

  • Smaller, more controlled studies show real improvement here: tadalafil improved exercise performance, NYHA functional class, and endothelial function in patients recovering from cardiac procedures (PMC; PubMed), and in an animal model it reduced heart failure severity after a heart attack. This is more mechanistically plausible (PDE5 inhibitors affect vascular tone and cardiac remodeling) but still limited to small trials or specific surgical populations, not general use.

Anti-inflammatory effects

  • Some research suggests tadalafil has mild anti-inflammatory activity alongside its vascular effects, which researchers think may partly explain some of its neurological and cardiovascular signals — but this is mechanistic/exploratory, not a clinical outcome that's been demonstrated to matter for patients.

Other proposed but not yet proven off label uses for Tadalafil

  • Sildenafil (a related PDE5 inhibitor) has documented off-label use for secondary Raynaud's phenomenon, female sexual arousal disorder, and altitude-induced hypoxemia. Tadalafil is sometimes assumed to work similarly given the shared mechanism, but for female sexual dysfunction specifically, the evidence is rated low quality — professional societies like ISSWSH don't include PDE5 inhibitors in their consensus guidance as of 2024, since there's no adequately powered confirmatory trial.

  • Some suspect that Tadalafil may also be shown beneficial in exercise tolerance and recovery due to the vascular smooth muscle relaxation and blood vessel widening. .

Why all the caution with the Studies?

Men who are prescribed and keep refilling ED medication may be healthier, more sexually active, wealthier or more engaged with their health care than men who are not. That is the problem and those factors are hard to fully adjust for in studies. Harvard Health Publishing puts it simply: these studies show a link, not that the drugs necessarily cause the benefit, and more research is needed [14]. For now, tadalafil should not be taken specifically for these unproven conditions.

Dosing at a Glance (Canada)

The dosing below follows the Health Canada–approved product monographs for Cialis [17] and Adcirca [19]. Canadian recommendations differ in a few ways from the U.S. label. The main differences are a higher standard as-needed dose (20 mg) and a higher daily starting dose (5 mg).

Indication and Recommended dose in Canada

ED – on-demand ("standard")

  • 20 mg taken before anticipated sexual activity, with or without food. The dose may be adjusted based on tolerability and effectiveness. Maximum once per day. Works from about 30 minutes up to 36 hours after a dose [17].

ED – once-a-day

  • 5 mg at about the same time each day, regardless of the timing of sexual activity. The dose may be lowered to 2.5 mg daily if needed for tolerability. Steady blood levels take up to 5 days [17].

BPH symptoms

  • 5 mg once daily at about the same time each day [17].

ED + BPH together

  • 5 mg once daily at about the same time each day [17].

PAH (Adcirca)

  • 40 mg once daily, taken as two 20 mg tablets together. Do not divide the dose over the day. No adjustment needed over age 65 [19].

The 10 mg and 20 mg Cialis tablets are meant for on-demand use and are not recommended for continuous daily use [17].

Special situations that need special treatment

Kidney or liver impairment, or strong CYP3A4 inhibitors (such as ritonavir or ketoconazole), for ED on-demand:

  • Avoid daily use of 10 mg or 20 mg.

  • Consider starting at 10 mg before sexual activity, no more often than every other day and no more than 3 times a week.

  • If 10 mg is tolerated but not effective enough, it may be increased to 20 mg. If 10 mg is not tolerated, stop on-demand dosing.

The same conditions, for once-a-day dosing (ED, BPH or ED/BPH):

  • 5 mg daily may be considered, lowered to 2.5 mg daily if needed.

  • Once-a-day use is not recommended with severe kidney impairment (CrCl under 30 mL/min) or severe liver impairment (Child-Pugh C).

On Alpha-blockers:

  • For ED on-demand, no dose change is needed with tamsulosin or alfuzosin. With doxazosin, start at 10 mg.

  • No adjustment is needed for once-a-day ED dosing.

  • Patients should be stable on the alpha-blocker before starting tadalafil.

  • Tadalafil is not recommended in combination with alpha-blockers for BPH.

Pulmonary Arterial Hypertension: (Adcirca):

  • With mild to moderate kidney impairment (CrCl 30–80 mL/min), start at 20 mg once daily and increase to 40 mg if tolerated. Not recommended when CrCl is under 30 mL/min.

  • With mild to moderate cirrhosis, start at 20 mg once daily after a careful benefit–risk assessment. Not recommended with severe cirrhosis.

  • Not recommended with potent CYP3A4 inhibitors such as ketoconazole, itraconazole or ritonavir

Safety Must-Knows

  • Daily dosing is Gentler of you and your body with lower incidence of headache, congestion, back pain, muscle aches, nasal congestion and flushing.

  • Never combine with nitrates in any form: nitroglycerin spray, tablets or patches, isosorbide, or amyl nitrite ("poppers"). In Canada, tadalafil should not be prescribed to anyone who has a nitrate prescription, even if they have not used it. If a nitrate is needed in a life-threatening emergency, at least 48 hours should pass after the last tadalafil dose [17]. Anyone who gets chest pain after taking tadalafil should seek emergency care and tell responders they took it.

  • Do not combine with Riociguat, a medication used for pulmonary hypertension [17].

  • Do not use tadalafil if you have ever had NAION, a sudden loss of vision caused by reduced blood flow to the optic nerve [17].

  • Adcirca (Used in PAH) should not be combined with Cialis or other ED pills [19].

  • Heart health check: tadalafil was not studied in men with a heart attack in the past 90 days, unstable angina, significant heart failure in the past 6 months, uncontrolled arrhythmia or blood pressure, low blood pressure (under 90/50), or a stroke in the past 6 months [17].

  • Heavy drinking can add to the blood-pressure-lowering effect and cause dizziness on standing [17].

  • Seek urgent care for an erection lasting 4 hours or more, or for sudden changes in vision or hearing [17].

  • Watch out for "herbal" sexual enhancement supplements, which have repeatedly been found to contain hidden PDE5 inhibitors or related compounds [18].

The Bottom Line

Tadalafil is a well-studied, versatile medication with strong evidence for erectile dysfunction, BPH urinary symptoms (especially when ED is also present), and pulmonary arterial hypertension. Its long half-life allows a flexible 36-hour as-needed window or a low-dose daily routine. In Canada, the standard on-demand dose is 20 mg, and the daily dose is 5 mg, lowered to 2.5 mg if needed. The daily routine comes with fewer side effects, removes the need to plan intimacy, and treats both ED and prostate symptoms with one pill.

Tadalafil’s cognitive, cardiovascular, anti-aging, anti-inflammatory, and improved exercise capacity has not been shown definitively in studies to date, but ongoing studies may show further benefits of this remarkably beneficial and diverse medication beyond the erectile dysfunction that it was first promoted for.
